New Employee Packet
When completing the paperwork please keep the following information in mind. Read carefully, follow all instructions. Be sure to answer all questions. Use blue or black ink only. DO NOT use pencil. Use your legal name as printed on your social security card. DO NOT use a nickname. A staff member from Payroll, Recreation or HR, must copy or view your Social Security Card. If you do not have one, please apply for one. The Social Security Administration will provide you with a letter for your employer to verify you have applied for a card. Please bring this letter with you when turning in your application. Please bring all completed paperwork along with your Social Security Card, License (if required) and your forms of ID required for the City to complete your I9 Form.

- IRS Form W-4 (2012) - If you want to exempt yourself from federal tax, when completing the W-4 (Employee’s Withholding Allowance Certificate) leave boxes 5 and 6 BLANK and write EXEMPT in box 7. This is the ONLY way to EXEMPT from paying federal taxes.

CIVIL SERVICE EXAMINATION NOTICES
When applying for a Civil Service examination, YOU MUST complete a Civil Service application form only obtainable at Parma Memorial Hall, 6617 Ridge Road, Parma, or the NAACP, 2131 Stokes Blvd., Cleveland.

The Human Resources and Purchasing Department is comprised of and
responsible for several important functions for the City of
Parma. The Purchasing area includes general supervision over
the purchases and contracts for all supplies and services
for the City. The purchasing employees maintain our office
supply storeroom inventory and ensure that all employees have
necessary office supplies on time. This Department is responsible for general supervision over the purchases and contracts for all supplies and services for the City and ensuring compliance with our informal and formal bid processes. In turn, this enables the City to obtain the best and lowest prices on all the materials, equipment and services utilized. The purchasing staff nurtures and develops relationships with local businesses to support our local economy.

The Human Resources area is responsible for the proper recruitment,
employment and retention of employees; administration of all
employee benefits including health insurance, life insurance,
retirement benefits, deferred compensation, flexible spending
plans and our employee assistance program. In addition, human resources
employees are responsible for administration of our worker's
compensation program and proper retention of all employee
records. The Human Resources staff coordinates training and other
employee HR programs.
This Human Resources and Purchasing Department sorts, delivers
and meters over 300 pieces of mail delivered to and sent from
the City Hall each day. In addition, our staff handles the
main telephone line into City Hall.

Tom Weinreich brings more than 25 years experience in various governmental positions, among them 12 years as a Parma Assistant City Prosecutor, eight years as Parma Tax Commissioner and two years in this same position as Parma Director of Purchasing & Personnel.
Past Secretary and member of the Board of Trustees for the Parma Area Chamber of Commerce, Weinreich also serves as Truancy Court Magistrate for Parma City Schools and is a former member on the Parma Municipal Income Tax Board of Review. He serves as well on the City of Parma Economic Development Advisory Council.
Weinreich graduated from Cleveland State University in 1992 with a bachelor's degree in Political Science. In 1998, he earned a law degree from the University of Akron School of Law. He is a member of The State Bar of Ohio.
Weinreich lives in Parma with his wife, Leanne and 17-year-old son Zach.
