Where do my tax dollars go?

Certified birth certificates are available for anyone born in Ohio. Certified death certificates are available for only those who passed away in Parma. The cost per birth or death certificate is $25.

1. In person: Arrive no later than 3:45 pm for same-day service at Vital Statistics, second floor of Parma City Hall. Cash, Visa and MasterCard are acceptable methods of payment. Vital Statistics will also accept a personal check accompanied by an Ohio driver's license. The address on the check must match the address on the driver's license.
   
   
2. By phone: Call Vital Statistics at 440-885-8816. Certificates are mailed using the U.S. Postal Service. Visa or MasterCard will be the only accepted form of payments for phone requests.
   
   
3. By mail: Send a letter stating the following:

• Full name at birth
• Date of birth
• Name of hospital along with the county where born
• Mother's full maiden name and the state or country where she was born
• Father's full name and the state or country where he was born
• Number of certificates requested

Payments for mailed requests must be in the form of a money order or bank check, made payable to the City of Parma. Personal checks will not be accepted and will be returned. Please include a self-addressed, stamped envelope. Mailed requests are processed and returned the same day they are received.
